MEMBERSHIPS

  • INCLUDES:

    • All concierge patient care provided by IFM Certified Family Practice DO, Dr. Brighton Miller.

    • Full access 24/7 to Dr. Brighton Miller via her cell phone

    • Dr. Miller can come to your house for appointments if desired

    • Unlimited appointments annually

    • Initial appointment is 1-hour, in-person with the option of having subsequent appointments via phone if desired

    • Receive 20% off all R+R Therapies. This discount applies to any immediate family members under the age of 19.

    • Lab Requisition follows with customized lab studies based on practitioner's individualized evaluation. Please note: Because this is a non-member option, a discount is not offered on labs.

    • Lab review Appointment upon receipt of lab results with a personalized protocol based on laboratory results.

    • Additional Follow-Ups as necessary provided but not included in this pricing. Call 817-720-6220 to book follow-up appt.

    LAB TESTING APPOINTMENT PACKAGE

Please notify us at least 24 hours in advance in the event you need to cancel or reschedule your appointment. Cancellations or rescheduling requests made less than 24 hours in advance will result in a $100 ($75 for peds) fee. Patients who fail to show up for their scheduled appointment without prior notice will also be charged the $100 fee.

This policy applies to all appointment types including acute visits, well visits and follow-ups. We appreciate your understanding and cooperation. If you need to cancel or reschedule, please call our office at 817-720-6220 ext 2.
